  Our Mission   Provision of Care   Who pays for Hospice care?  
  Nurses Plus Hospice affirms the Circle of Care and its commitment to optimize available resources and its interdisciplinary careging team to provide safe, cost-efficient but individualized and ethical care to patients and their caregivers reflecting the spirit and philosophy of compassionate caring that emphasizes the comfort and dignity of patients int their last phases of life.  
Nurses Plus Hospice delivers care in the most comfortable environment for the patient.
     
  -Routine Home Care - Intermittent care provided in the patient's own residence or any place the patient considers home.
  -Continues Care - Consists of hospice care for a minimum of 8 consecutive hours. This level is utilized when patients require more than routine home care.
  -In-patient Care - Utilized for the management of acute symptoms which cannot be controlled in the home. In-patient care is to be delivered in "contract beds" located in the hospitals, Skilled Nursing Facilities or sub-acute facilities.
  -Respite Care - Provides a brief respite (up to five consecutive days) for the primary caregivers. Care is delivered in an institutional settings

 

Hospice care is covered by most insurance plans including Medicare (PArt A), Medi-Cal and private payors with some, or no out of pocket cost to the patient.
