When patients are faced with a terminal condition, choices for care are limited. Nurses Plus Hospice offers an alternative form of care by working closely with physicians to provide palliative and end of life care.
Patients may be eligible for hospice care if:
The patient's Attending Physician nd Medical Director/ Hospice Physician certifies that the patient has a limited life expectancy of 6 months or less if the terminal illness runs its normal course.
Hospice care is selected by patient or the patient's legal representative if the patient is incapable of making healthcare decisions.
Hospice care is covered by most insurance plans including Medicare (Part A), Medi-Cal and private payors with some, or no out of pocket cost to the patient.

Nurses Plus Hospice affirms the Circle of Care providing a holistic continuum of care through its interdisciplinary team, providing individualized and ethical care to meet the physical, emotional, spiritual and practical needs of patients in their last phases of life.
Nurses Plus Hospice provides care reflecting the spirit and philosophy of caring that emphasize comfort and dignity for the dying.
We provide hospice care not solely to the patients, but also for their families and/or caregivers providing ongoing support to assist with the grieving process through its Bereavement Services.
Services
Nurses Plus Hospice provides the following services through its Interdisciplinary Team:
ATTENDING PHYSICIAN

Remains the patient's primary care Physician and works as a member of the Hospice team.
MEDICAL DIRECTOR / HOSPICE PHYSICIAN

Leads the interdisciplinary team in developing the plan of care and provides consultation to the attending Physician.
SKILLED NURSES
Evaluates the patient's condition, coordinates and/or implements the plan of care, performs medical treatments, pain management and symptom control. Provides family and caregiver support.
CHAPLAIN / SPIRITUAL COUNSELOR

Provides and/or oversees the spiritual care of patient and/or family. Works as liaison with clergy of individual's choice. Provides bereavement support, assistance with funeral and memorial preparations.
SOCIAL WORKER

Assess social service needs of the patient and family. Provides counseling, assists in coping with anxiety, fear, stress, family conflicts, financial needs, depression, and concerns about death and the bereavement process. Assist with community resources and end of life preparations.
HOME HEALTH AIDE
-
Provides direct personal care and assists patient/family with other activities of daily living under the supervision of the RN Case Manager.
VOLUNTEER
-
Provides companionship and a wide-range of support to patients and their families. Assists with bereavement needs.
